- fever
- chills
- new or worsening cough
- shortness of breath
- sore throat
- tested positive for COVID (or are awaiting the results of a COVID test)
- Please sanitize your hands immediately upon entering the office.
- If you do not have your own mask you will be provided one upon entering the office for the duration of your visit.
- Medical grade masks are still required, and will be provided at the office.

To our valued patients
As we are a very busy clinic, we require unneeded appointments to be cancelled with no less than 24 hours notice.
Missed appointments may result in a fee that will be charged to the patient. However, due to COVID-19 we will make exceptions if you have developed any respiratory symptoms and need to cancel with less notice.
